An In Case of Emergency (ICE) card is available from GoldenMaturity.com which includes a pocket sized form to state your medical history, the name of your doctor and emergency contacts and to advise the hospital about your DNR/Proxy/LivingWill. You can call Annette Fisher 718-968-5242 for information about getting free cards for distribution to your organization or to advertise on the cards.
